CONSENT TO THE PROCESSING OF PERSONAL DATA

Prepared in accordance with Article 9 of Federal Law No. 152-FZ “On Personal Data” dated 27.07.2006 (Russian Federation). This is an English-language working translation for the Client’s understanding; the document actually signed and relied upon should be in the form confirmed with a lawyer, typically bilingual or in Russian.
By submitting a contact, inquiry, or order form on the website russianroots.su, and/or by ticking the consent checkbox displayed at the point of collection, I (hereinafter the “Data Subject”) freely, of my own will and in my own interest, give my unconditional consent to Elena Pavlovna Tepukhina, an individual providing services under Taxpayer Identification Number (INN) 421813397220, operating the website russianroots.su (hereinafter the “Operator”), to process my personal data on the following terms:
1. Purpose of processingResponding to my inquiry and performing the service agreement for genealogical research concluded between myself and the Operator, including: submitting official requests to archives, civil registry offices, and other institutions; preparing and delivering research results to me; communicating with me regarding the services; issuing invoices and receiving payment.
2. Personal data covered by this consentThis consent covers the personal data I provide through the form(s) on the website, in particular:
● full name;
● email address;
● phone number (if provided);
● country of residence;
● the content of my inquiry and any information about my relatives/ancestors that I provide for the purposes of the research (full names, dates, places of birth or residence, and other genealogical information);
● identity document details and residential address, where I separately provide these for the purpose of issuing a power of attorney;
● any other field displayed on the specific form at the time of submission on russianroots.su.
3. Actions covered by this consentThis consent covers the following actions with personal data: collection, recording, systematization, accumulation, storage, clarification (updating, amending), retrieval, use, disclosure (provision, access) to third parties in the cases set out in Section 4 below, de-identification, blocking, deletion, and destruction of personal data.
4. Disclosure of personal data to third partiesI agree that, for the purposes stated in Section 1, my personal data may be disclosed to: archival institutions, civil registry offices, and other organizations holding relevant records; a notary — for the purpose of issuing a power of attorney; a translator — where translation of documents is required; a research assistant engaged by the Operator to help carry out archival inquiries, acting under an obligation of confidentiality. A separate Consent to the Transfer of Personal Data to Third Parties sets out these recipients in more detail.
5. Method of processingPersonal data is processed both with and without the use of automation tools (mixed processing), including storage in electronic and/or paper form.
6. Validity periodThis consent is valid from the date of signing for the entire term of the service agreement referred to in Section 1, and for [3 (three)] years thereafter, unless otherwise required by Russian law.
7. Withdrawal of consentThis consent may be withdrawn at any time by sending the Operator a written request to [insert contact email], or by any other means allowing confirmation of receipt. Upon withdrawal, the Operator will cease processing the personal data and destroy it within 30 (thirty) days, except where continued processing (storage) is required by Russian law.
This consent takes effect from the moment I submit the form and/or tick the checkbox, and remains valid for the period described in Section 6 above, unless withdrawn earlier as described in this Section 7.
