CONSENT TO THE TRANSFER (DISCLOSURE) OF PERSONAL DATA TO THIRD PARTIES

Prepared in accordance with Articles 6 and 9 of Federal Law No. 152-FZ “On Personal Data” dated 27.07.2006 (Russian Federation). This is an English-language working translation for the Client’s understanding; the document actually signed and relied upon should be in the form confirmed with a lawyer, typically bilingual or in Russian.

By submitting a contact, inquiry, or order form on the website russianroots.su, and/or by ticking the corresponding consent checkbox, I (hereinafter the “Data Subject”), having given my general Consent to the Processing of Personal Data to Elena Pavlovna Tepukhina, an individual providing services under Taxpayer Identification Number (INN) 421813397220, operating the website russianroots.su (hereinafter the “Operator”), additionally and specifically give my consent to the transfer (disclosure) of my personal data, and of the personal data of relatives that I provide for the purposes of the research, to the following third parties:
1. Recipients of personal data and purpose of transfer●       Archival institutions, civil registry offices (ZAGS), libraries, museums, and other organizations holding relevant historical or civil records — for the purpose of submitting research requests and receiving documents and information.
●       A notary — for the purpose of issuing and certifying a power of attorney authorizing a representative of the Operator to submit requests to archives and registry offices on my behalf.
●       A professional or certified translator — where official or certified translation of documents obtained during the research is required.
●       A research assistant engaged by the Operator (currently: [insert name/status, e.g. a family member or subcontractor assisting with archival correspondence]) — for the purpose of preparing, submitting, and following up on archival inquiries, acting under an obligation of confidentiality to the Operator.

2. Scope of data transferredOnly the personal data necessary to achieve the purpose described for each recipient in Section 1 is transferred — in particular, full name, dates and places of birth/residence, identity document details (where a power of attorney is required), and genealogical information about relatives that I have provided.
3. Validity periodThis consent is valid from the date of signing for the entire term of the service agreement for genealogical research concluded with the Operator, and for [3 (three)] years thereafter, unless otherwise required by Russian law, or until withdrawn in accordance with Section 4.
4. Withdrawal of consentThis consent may be withdrawn at any time by sending the Operator a written request to [insert contact email], or by any other means allowing confirmation of receipt. Withdrawal does not affect the validity of disclosures already made before the Operator received the withdrawal request.
This consent takes effect from the moment I submit the form and/or tick the checkbox, and remains valid for the period described in Section 3 above, unless withdrawn earlier as described in this Section 4.
